Section	Information Required	Notes
Deponent Details	Your Name (W/o. Deceased), Age, Occupation, and Full Residential Address (Telangana State).	This is the person swearing the oath (the applicant).
Deceased Details	Late Husband/Wife’s Full Name, Father/Mother’s Name, Date of Expiry, and Place of Death.	Must match the Death Certificate.
